Employment related events:
New employment - This will require completion of forms such as the Participant Data Form, a choice of medical plan (Indemnity or HMO) and enrollment forms. Call your SWA HYPERLINK https://www.swadmin.com/locations.html" Area Administrative Office for the enrollment package for your Trust Fund.
Sick Leave/Disability - You may be eligible for an extension of benefits during a period of sick leave/disability. To find out if your Trust Fund coverage can be extended, call your SWA HYPERLINK https://www.swadmin.com/locations.html Area Administrative Office for information and the necessary forms.
Retirement- You may be eligible for Retiree health and welfare benefits. Call your SWA

Marital Status:
Marriage - You must enroll your spouse with the Fund. To enroll your spouse, complete a new Participant Data form and mail to the SWA Area Administrative Office with a recorded copy of your marriage certificate.
Divorce - You must notify the SWA Area Administrative Office immediately of any change in marital status. A divorced spouse is not eligible for plan benefits even though a court order is issued requiring you to provide health benefits. You will be held personally liable in the event you fail to notify the SWA Area Administrative Office of a change in marital status if a claim by your former spouse is mistakenly paid by the Trust Fund for services after that date.

Events Affecting Children:
Birth - Your new baby is covered from birth but in order to avoid delays in coverage or eligibility issues, you must enroll your baby with the Trust Fund. Complete the Participant Data Form and mail with a certified copy of the birth certificate to your SWA https://www.swadmin.com/locations.html
Area Administrative Office.
Age 19 -Your dependent children are covered up to their 19th birthday as long as they live in a parent/child relationship with you and are dependent upon you for support.
Student Status - Coverage under your medical plan may be extended for as long as your child maintains full-time student status and is dependent on you for support, but only through the age established by your Trust. A certification of school enrollment must be submitted at the start of each new session and a transcript of grades for the prior session showing the units completed must be submitted upon request.
Disabled Child - Coverage for your child who is incapable of self-sustaining employment because of a mental or physical handicap will not be terminated at age 19 if the child is chiefly dependent upon you for support, provided that written evidence of such incapacity is submitted to the Fund prior to your child reaching age 19.
