Things You Need to Do
Use this checklist to get the most from your Trust Fund benefits.
Initial enrollment documents may come from other sources for some Trust Funds.
1. Welcome Letter
By now you should have received a welcome letter along with a Participant Data Enrollment Form (PDF) and a comparison brochure outlining your benefit plan options. You may want to retain this letter for future reference, as it contains the official name of the Trust Fund under which you are covered.
2. Fill out an Initial Participant Data Enrollment Form
The Participant Data Enrollment Form (PDF) is the first step in activating your coverage. The form will require information on all of your family members with supporting documents to establish eligibility. If you need a PDF form, you can call SWA at (877) 350-4792 or stop by in person at the nearest SWA location.
You must fill out and return this questionnaire to SWA in order to be eligible for benefits.
3. Choose a Medical and/or Dental Plan
The comparison brochure is a quick reference to compare the various benefit plans available
to you and your family. Review it carefully and select one that best fits your needs.
